Clinical observation on Patients with Perforation of the Terminal Ileum


Abstract
A clinical observation was made of 261 cases of surgical patients who were operated with perforation of the terminal ileum secondary to salmonellosis which were experienced from Jan. ; 1966 to Sept.
1974 at the department of surgery, Choong Boo City Hospital, Jan., 1971 to Sept. 1974 at the depa¡þrtment of surgery, Sub. Dai Moon City Hospital and Oct., 1971 to Sept., 1974 at the department of surgery, Kyung Hee University Hospital.
The results were as follows:
1. The ratio of frequency between male and female were approximately 3:1. The age in the majority of the pateints was between, second and third decade of life (59. 0%).
2. In seasonal incidence 115(59.0%) of. the cases occured in Summer.
3. The most diagnostic symptoms and signs encountered were sudden lower abdominal pain with or without nausea and vomiting, tenderness and muscular rigidity mostly - after an irregular febrile episode.
4. The hematologic findings included: normal or leucopenia in 146 cases (71.6%) and normal or hypohemoglobinemia in 188 cases(96.9%).
5. Positive Widal test was observed in 128 cases (59.8%) out of 214 cases.
6. X-ray findings revealed subdiaphragmatic free gas in 167 cases (74.2%) and with signs of paral¡þytic ileus in 108 cases(48.0%).
7. Perforation was developed most frequently at second to third weeks after onset and the duration from perforation to operation was most frequent in second to third days.
8. In operative findings the site of perforation of 196 cases (75.1%) was within 40 cm from the ileocecal valve and the number was single in 204 cases (78.2%).
9. The operative procedures were simple closure for 184 cases (70. 1%), segmental resection Vth end to end anastomosis for 75 cases (28.7%) and resection with ileotrahsverse colostomy for 2 cases (0.8%) .
10. The most frequent complications were 155cases (59.4%) of wound infection, absces and disruption.
11. The overal mortality rate was 42 cases (16.5%) .
